ABSTRACT

Objective:To investigate the clinical features of drug-induced hypersensitivity syndrome (DiHS) and enhance the recognition of its diagnosis and treatment.Methods:Drug-induced hypersensitivity syndrome diagnosed in the Second Xiangya Hospital of Central South University in recent 6 years were retrospectively analyzed and summarized.Results:Of 35 cases of drug-induced hypersensitivity syndrome there were ten types of suspected drugs. Antibiotics were the most common allergy-inducing drug, followed by allopurinol, antiepileptics, and non-steroidal anti-inflammatory drugs. The expected clinical findings of DiHS were characterized by skin rash, fever, enlarged lymph nodes, which may be accompanied by eosinophilia, liver function damaged. The lesions of DiHS were variable and the majority of cases 65.71%(23/35) were characterized by eruptive papules.The main therapeutic approach of DiHS was systemic application of glucocorticoids and intravenous immunoglobulin (IVIG). When necessary its combination with immunosuppressant or hemoperfusion (HP).Conclusions:DiHS should be on the high alert when a patient with medication history, rash, high fever and lymph nodes large, accompanied by eosinophilia, liver function damaged. Predictors of good prognosis are early diagnosis and treatment.

Rosai-Dorfman disease (RDD) , also known as sinus histiocytosis with massive lymphadenopathy, is a rare benign self-limited histiocytic proliferative disorder with undetermined etiology. There is still no specific treatment for RDD. Surgical excision is the first-line treatment, while topical and systemic therapy can be chosen for the patients who are not unfit for surgery. Classic topical therapies include topical treatment or local injection with glucocorticoids. Photodynamic therapy and laser immunotherapy are considered as promising topical treatments. Traditional systemic treatments include oral glucocorticoids, immunomodulators, acitretin, and so on. Rituximab is a new treatment option for patients with CD20-positive RDD. This review summarizes new progress in the treatment of RDD in recent years.

ABSTRACT

Objective To investigate clinical effect of recombinant human growth hormone (rhGH)for inter-venting central precocious puberty in children,to provide reference for clinical treatment.Methods 80 patients with growth deceleration slows CCP were selected,when height growth rate below 4cm/year,40 patients on the basis of joint GnRHa used rhGH treatment were selected as the observation group,and the remaining 40 patients refused to use rhGH,continued to use of GnRHa treatment were selected as the control group.After treatment predicted height,height,bone age,growth rate and other indicators were compared.Results In observation group,the actual height Ht, predicted height PAH,the growth rate Gv,serum insulin -like growth factor -1 (IGF -1 )after treatment were (1 44.48 ±6.59)cm,(1 54.94 ±4.52)cm,(4.43 ±0.64)cm/6months,(1 32.25 ±8.84)ng/mL,compared to before treatment,the differences were statistically significant (t =6.548,P <0.01 ;t =5.734,P <0.01 ;t =28.869,P <0.01 ;t =20.65,P <0.01 ),compared with the control group,the differences were significant (t =3.943,P <0.01 ;t =4.759,P <0.01 ;t =28.247,P <0.01 ;t =20.882,P <0.01 ),there were no differences in other indices;hor-mones FSH,LH and other indicators of the two groups before and after treatment,and blood sugar,thyroid indicators showed no abnormality.Conclusion Recombinant human growth hormone in the treatment of central precocious puberty deceleration in children does not increase bone age,which will help improve the growth rate and predicted height,which worthy of clinical application.

ABSTRACT

The low butanol concentration of acetone-butanol-ethanol fermentation causes uneconomical product recovery. In this work, the effect of small molecule non-ionic surfactants on butanol fermentation was evaluated, using laboratory stocks of Clostridium acetobutylicum ATCC 824. Non-ionic surfactants substantially increased butanol production when additive amount was higher than 1% (W/W). Butanol concentration reached 16.9 g/L with 5% (W/W) Tween 80 and 100 g/L glucose in a 5 L fermenter. It was found that surfactants micelle solubilization capacity to butanol was very limited, indicating that butanol could hardly enter the surfactants micelle. Butanol production improvement was probably caused by cell surface hydrophobicity change due to surfactants adsorption.

ABSTRACT

Objective To investigate academic thoughts of physicians in the treatment of dryness syndrome in Jin, Yuan, Ming and Qing Dynasties. [Methods] With learning the ancient medical works, etiology-pathogenesis and principle-method-recipe-medicines of dryness syndrome were analyzed, and characteristic of the treatment of dryness syndrome was summarized. [Results] Liu Wansu summarized that blood exhaustion and body fluid deficiency were the pathology of dryness syndrome, and zang and fu organs and skin dry manifestation were the clinical symptoms, and he proposed 11 prescriptions. Yu Jiayan put forward the name of autumn-dryness disease, summarized clinical characteristics was thirsty. He proposed the rule to treat the dryness syndrome from lung and liver, and also created Qingzao Jiufei decoction to treat the warm and dryness pathogen attacking lung syndrome. Fei Boxiong raised dryness syndrome could be divided into warm dryness and cool dryness, treated from five zang organs, and emphasized the application of harmonization method to mild treatment. Ye Tianshi raised dryness syndrome could be divided into internal injury and exogenous disease, to treat upper dryness from QI and lower from blood based on Wei qi camp blood and Sanjiao syndrome differentiation . Wu Jutong treated autumn-dryness in Sanjiao syndrome differentiation, and pointed out the progression of autumn-dryness could be along upper jiao, middle jiao and lower jiao. He created Xingsu powder, Sangxing decoction and Shashen Maidong decoction. [Conclusion] There were al kinds of innovation of syndrome differentiation and treatment of dryness syndrome in Jin, Yuan, Ming and Qing Dynasties, and it is instructive to diagnosis and treatment for later physician.

ABSTRACT

Objective To analyze the clinical and nursing care of peripartum reversible posterior leukoencephalopathy syndrome (RPLS).Methods Thirty five cases with peripartum RPLS were retrospectively reviewed.Results All patients developed acutely,22.9% in the antepartum period and 77.1% in the postpartum period.The major clinical characteristics were headache (65.7%),seizures(62.9%),visual disturbances (54.3%),altered mental status (22.9%).The most common abnormality on neuro-imaging was edema predominantly involving the cortex and subcortical white matter in the posterior portions of the cerebral hemispheres,including the occipital lobes (77.1%),parietal lobes (65.7%),temporal lobes (28.6%) and frontal lobe (20.0%).MR diffusion-weighted imaging and ADC mapping revealed vasogenic edema.The treatments included aggressive blood pressure control and anticonvulsants with special nursing care.The clinical symptoms and the lesions on neuroimaging improved or resolved within 1 day to 2 months.Conclusions The prognosis for peripartum RPLS is ensured as long as the diagnosis is made properly and reasonable application of care program.

ABSTRACT

ObjectiveTo explore the expression and clinical significance of Ras-related C3 botulinum toxin substrate 1 (Rac-1) protein and hypoxia-inducible factor 1α (HIF-1α) in breast cancer and their relationship with tumor stage, node metastasis status and hormone receptor status. MethodsImmunohistochemical method was used to detect the expression of Rac-1 and HIF-1α protein in 139 specimens of breast cancer,combined with their clinical pathological characteristics and hormone receptor status for statistical analysis.Results The expression of Rac-1 and HIF-1α in breast cancer tissue were 64.75 % and 65.47 %,respectively.The levels were related with the stage of TNM and histological grade (P < 0.05,P < 0.01).There was no significant correlation between these two proteins'expression and the age of patient,primary pathological location, histologic type and menstruation status of breast cancer patients(P > 0.05). The expression of Rac-1 and HIF-1α had no statistical significance between triple negative breast cancer(TNBC)and non-triple negative breast cancer (NTNBC) patients (x2 =1.1229,x2 =0.1786,P > 0.05); The expression level of Rac-1 was positively correlated with HIF-1α in these breast cancer specimens (rs =0.414,P < 0.01).ConclusionThe expression of Rac-1 is positively correlated with HIF-1α level in breast cancer.The relationship between Rac-1 and HIF-1α might be as a new important marker to estimate the invasion,metastasis extent and prognosis in breast cancer.Blocking the regulation between Rac-1 and HIF-1α might be a new treatment target in breast cancer.

It has been found that carrying out case-teaching method in biochemistry teaching is good for students to arouse study interest and enhance the abilities of autonomic learning, question-analyzing and question-solving as well as consolidate theory knowledge. It is worth generalizing as a new teaching method for biochemistry in medical college.

ABSTRACT

ObjectiveTo determine the expression of matrix metalloproteinase-2 (MMP-2),cysteine rich protein 61 (Cyr61) and CD25 in esophageal carcinoma and investigate their clinical significance.MethodThe expression of MMP-2, Cyr61 and CD25 in 68 cases of esophageal tumor tissues (esophageal carcinoma group ) and part-cancer( para-cancer group ) and 68 cases of reflux esophagitis (control group ) was detected by immunohistochemical method and the results were statistically analysed. ResultsThe positive expression rates of MMP-2,Cyr61 and CD25 were 82.35%(56/68), 73.53%(50/68) and 89.71%(61/68)respectively in esophageal carcinoma group,there were significant differences compared with those in paracancer group and control group(P < 0.05) ; while there was no significantdifference between para-cancer group and control group (P > 0.05 ). The positive expression rates of MMP-2, Cyr61 and CD25 in esophageal carcinoma group in different T stage, N stage and M stage all had statisticall differences (P < 0.05),while there was no significant difference in different degree of differentiation (P > 0.05 ). ConclusionMMP-2,Cyr61 and CD25 is highly expressed in esophageal carcinoma,there is some significance for understanding the occurrence, development and prognosis of esophageal carcinoma.

ABSTRACT

AIM: To investigate the effect of puerarin on diabetic myocardial damage and to explore its possible mechanisms in rats. METHODS: The male Sprague-Dawley rats were randomly divided into normal control group, diabetic group, high dose puerarin (160 mg/kg), middle dose puerarin (120 mg/kg), low dose puerarin (80 mg/kg) treatment groups and aminoguanidine (100 mg/kg) treatment group. Corresponding drugs were intraperitoneally injected once a day. The animals in normal control group and diabetic group were given equal propylene glycol. 12 weeks later, the rats were sacrificed and their cardiac muscles were collected. Myocardial structure was observed under transmission electron microscope (TEM). The blood glucose concentration and the activities of superoxide dismutase (SOD), Ca~(2+)-ATPase, Na~+-K~+-ATPase and the content of malondialdehyde (MDA) in myocardial homogenate were measured biochemically. In addition, the mRNA expressions of peroxisome proliferator activated receptor γ (PPAR-γ), glucose transporter-4 (GLUT-4) and aldose reductase (AR) in myocardial tissues were determined by reverse transcription polymerase chain reaction (RT-PCR). RESULTS: It is observed that the myofibrils were diminished, broken or fused, some lipid droplets deposited in the cytoplasm of cardiocytes and part of cristae of mitochondria were broken or disappeared under TEM in diabetic group. The activities of SOD, Ca~(2+)-ATPase, Na~+-K~+-ATPase as well as the mRNA expressions of PPAR-γ and GLUT-4 decreased significantly (all P<0.01). The blood glucose concentration and the MDA level and the AR mRNA expression increased obviously (all P<0.01) in diabetic group as compared to those in normal control group. How-ever, in puerarin treatment groups, the above changes were reversed, a significant differences of those were found as compared to those in diabetic group (P<0.05 or P<0.01, respectively). The pathological change of cardiac muscles was relieved. It showed that myofibrils were well-arranged and only few lipid droplets deposited in the cytoplasm of cardiocytes and most mitochondria had clear and regular cristae under TEM in puerarin treatment groups. CONCLUSION: Puerarin exerts preventive and remedial effects on the diabetic myocardium, which may be related to up-regulating the mRNA expressions of PPAR-γ and GLUT-4, promoting glucose uptake and relieving oxidative stress damage.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the role of thrombopoietin (TPO) in the pathology of chronic thrombocytopenic disease.</p><p><b>METHODS</b>We measured the endogenous plasma concentration of TPO in 40 patients with acquired aplastic anaemia (AA) and in 32 patients with idiopathic thrombocytopenic purpura (ITP) by a sensitive Sandwich enzyme-linked immunosorbent assay (ELISA) and compared the results.</p><p><b>RESULTS</b>Plasma TPO concentrations were significantly higher in AA patients (774 +/- 393 pg/ ml) in comparison with healthy control subjects (55 +/- 34 pg/ml, P < 0.001), and there was a significant negative correlation between their plasma TPO concentrations and platelet counts. In patients with ITP, however, the TPO levels were normal or only slightly elevated (73 +/- 36 pg/ml), and there was no significant difference compared with healthy controls (P > 0.05). There was also no relationship between their plasma TPO levels and platelet counts.</p><p><b>CONCLUSIONS</b>TPO levels may be regulated not only by platelets but also by megakaryocytes in AA and ITP, and measurement of TPO levels is useful for diagnosing thrombocytopenia and understanding the pathophysiology of thrombocytopenia.</p>

AIM:To investigate the protective effect of puerarin on testis in diabetic rats.METHODS:The male Sprague-Dawley rats were randomly divided into normal control group,diabetic group,high dose puerarin(160 mg/kg),middle dose puerarin(120 mg/kg),low dose puerarin(80 mg/kg)treatment groups and aminoguanidine(100 mg/kg)treatment group.Corresponding drugs were intraperitoneal injected once a day.The animals in normal control group and diabetic group were given equal propylene glycol.12 weeks later,the rats were sacrificed and their testes were collected.Testicular structure was observed under transmission electronic microscope(TEM).The contents of nitric oxide(NO),malondialdehyde(MDA)and the activity of nitric oxide synthase(NOS),inducible nitric oxide synthase(iNOS),superoxide dismutase(SOD),Ca2+-ATPase and Na+-K+-ATPase in testis homogenate were detected biochemically.The levels of endothelin-1(ET-1)and calcitonin gene-related peptide(CGRP)were also measured by radioimmunoassay.In addition,the mRNA expressions of eNOS,iNOS,CGRP,ET-1 and AR in testis tissues were determined by reverse transcription polymerase chain reaction(RT-PCR).RESULTS:The pathologic changes such as cytoplasmic inclusion of sertoli cell and obvious vacuoles between sertoli cells and basal membrane of the contorted seminiferous tubules were observed under TEM in diabetic group.The contents of NO and CGRP,the activities of SOD,Ca2+-ATPase,Na+-K+-ATPase as well as the mRNA expressions of eNOS and CGRP decreased significantly(P

ABSTRACT

AIM: To explore the activative effect of water extract from cultured mycelium of Paecilomyces cicadae (P. cicadae) on peritoneal macrophages (PM?) and alveolar macrophages (AM?) of rats.METHODS: The rats were randomly divided into four groups: normal control group(Ⅰ), cyclophosphamide (Cy) group (Ⅱ), P. cicadae group(Ⅲ), Cy+ P.cicadae group(Ⅳ).The rats were bred in the same circumstance and were administered with corresponding drugs by subcutaneous injection for 26 days. PM? and AM? of rats were irrigated by normal saline and collected by centrifuge and incubated in a humidified incubator for 2 h at 37 ℃. The activities of acid phosphatase (ACP) and lactate dehydrogenase (LDH) in the PM? and AM? were determined by biochemical methods, and the capability of PM? and AM? in phagocytosis of neutral red were measured by colorimetric method too. RESULTS: After administering P. cicadae to rats, the activities of ACP and LDH in PM? and AM? of normal rats were elevated significantly,and the reduction of the activities of ACP and LDH in PM? and AM? of rats due to Cy was notably antagonized, and the capability of PM? and AM? phagocytizing the neutral red were strengthened significantly.CONCLUSION: P. cicadae can activate the PM? and AM? of rats.
